We have very strong community links with our local care partnership.

Frailty Proactive Team

The Proactive Care Team are part of Seacroft PCN which support four GP practices in Seacroft and the neighbouring areas.

The Proactive Care Team first came together at the beginning of 2021 to help support older people in this area to live well.

Our main focus is to have good conversations with people and understand what’s important to them and whether they need support at this time.

We understand this is a time of great hardship for many people and as we get older situations can become more complicated and difficult to understand; for some people it can be hard to access the help they need if they don’t have anyone close to ask.

We work across and together with other organisation in the area to help support people with their health, to live well in their community and aim to improve their experience and outcomes.

We are part of Seacroft Local Care Partnership.

Clinical Pharmacy Team

The Seacroft PCN pharmacy team aims to improve outcomes for patients from their medication and reduce the risk of harm; this is done by performing thorough structured medication reviews and through shared decision making, working with the patient to ensure the regime they are prescribed is optimum and individualised for them.

The patient populations we focus on are those most at risk of harm from their medication; this includes all care home residents, frail patients in their own homes, multimorbid patients on polypharmacy or those on high-risk medication.

The PCN Pharmacy team works closely with the PCN proactive/ frailty team and where beneficial, we can arrange for joint home visits for a holistic review of the patient, part of which will involve reviewing the medication.

We also support all the care homes with medicines management and work with them to ensure CQC requirements are being met.

Patients who have had a hospital stay where the admission has been lengthy or had multiple medicines changes can also be referred to us so we can support them after discharge, ensuring the changes have been understood and give them opportunity to ask questions about their new regime.

Complex prescribing enquiring from GPs/ nurses/ patients within then PCN can also be researched and answered.

Where possible we aim to learn from best practice for medicines related policies from individual practices within the PCN and try to align them to ensure we work consistently as a PCN.

Some of the medicine’s aspects of the NHS’ investment and impact fund are also delivered by the PCN Pharmacy team.

Children’s Asthma Practitioner

The Seacroft PCN team also has access to a Childrens’ Asthma Practitioner.

Asthma can be a lifelong condition but the goal of treatment is to achieve a normal quality of life.

If you suspect that your child has asthma, it is important to see a doctor; asthma requires careful management and it is vital that you know what to do in the event of a serious asthma attack.

Extended Access

Our Extended Access Services and appointments are held at our Foundry Lane and Oakwood Lane practices, please contact the practices for further details.

First Contact Practitioner

Seacroft PCN’s team also offer First Contact Practitioner services.
They can help patients with musculoskeletal issues such as back, neck and joint pain by:

  • assessing and diagnosing issues
  • giving expert advice on how best to manage their conditions
  • referring them onto specialist services if necessary.

Patients with back and joint pain, including conditions such as arthritis, will now be able to contact their local physiotherapist directly, rather than waiting to see a GP or being referred to hospital.

Linking Leeds

The Linking Leeds Social Prescribing service operates out of Community Links which has led a consortium of local partners to deliver this service from the 1st of September 2019, commissioned by NHS Leeds CCG.

Our partnership has more than ten years of collective experience delivering Social Prescribing to the people of Leeds. We work alongside Leeds City Council and Leeds GP Confederation.

Referrals can be made via your GP Surgery, please contact your surgery to learn more or to arrange a referral.
